Parents' project - questionnaire

The BSA is currently preparing a resource to help the parents of children who stammer to ensure that their child has the best opportunities possible at school. (More on Parents' Project...)

Therefore we would be very grateful for any information that you are willing to provide about your experiences at the different stages of your child's schooling. You may complete the form without giving your contact details, but it would be very helpful for us if you could supply a phone number where we could contact you to follow up your answers in more detail. If you prefer, you can print off or email in the questionnaire as a Word document.
